To use the Evaluation Tools or the web-based data entry and reporting system, we ask you to register with us. By registering with us, we can keep an accurate count of who is using the tools. We will not share your registration information with anyone. The use of the evaluation forms is free; however, we ask that you give credit to the authors on any reports using the data generated from the evaluation tools. We also encourage you to send your data to us to include in the national data base.

If you choose to use the web-based data entry and reporting system, there is a technical assistance fee to: (1) support the maintenance of the system, (2) provide on-demand technical assistance, and (3) provide funds for further development of new tools. Below shows the pricing for using the system for one year. Pricing allows for one login with as many groups wished by the user. Users needing more than one login or modifications (i.e., changes to the forms) will need to communicate needs and requirements before pricing can be quoted. In addition, pricing below is for the base system that includes: Skills for Everyday Living; Communication; Critical Thinking; Decision Making, Goal Setting, Leadership, and Solving Problems. Present plans are to add new forms for younger ages that will include interactive web-based screens which is not included in the pricing below. Discounts for using the interactive system will be given to users of the base system. Due to the major contributions by Pennsylvania State University Extension and Purdue University Extension, University Extensions will receive a discount for using the base system.
